WHAT IS AGE WELL SANTA CRUZ COUNTY?

The Human Services Department is dedicated to the well-being of older residents and people with disabilities throughout Santa Cruz County.

The Age Well Santa Cruz County initiative will allow us to develop our county's blueprint for a local Master Plan on Aging. This plan will help us ensure people of all ages and abilities in our county are engaged, valued, and afforded equitable opportunities to thrive as they age, how and where they choose.

COMMUNITY NEEDS ASSESSMENT SURVEY

As part of Age Well Santa Cruz County and designing our Master Plan on Aging, please take the Community Needs Assessment Survey to help us collect community feedback focused on aging in the county.

REQUEST A PAPER SURVEY

If you would like us to mail a paper survey to your home address so you can fill it out and mail it back to us, please call us at (831) 454-4397 or send an email to: AgeWell@santacruzcountyca.gov with "Mail Me a Paper Survey" in the subject line. Tell us your first and last name, your mailing address, what type of survey you are requesting (English, Spanish, Large Format in English), and how many copies you need of each.

Return Your Paper Survey!

You can drop your completed paper survey in any Age Well Survey Return Box.

You can find survey return boxes at all county library branch locations, at the London Nelson Community Center in Santa Cruz, at the Watsonville Senior Center, at all Dientes Dental Care offices, and at many other locations around the county. For assistance finding additional survey return box locations, call us at 831-454-4397.
